Composition

Why is there such a high incidence of this condition?

The vagina and its healthy microbial flora form a balanced ecosystem of the vagina, where the environment controls local bacterial strains and the flora, in turn, controls vaginal environment and provides a natural protection of mucosa against pathogens. Sufficient levels of oestrogens are required for maturation of intact vaginal epithelium, as well as for the growth of Lactobacteriaceae (LB), which, in turn, produce lactic acid to maintain normal levels of vaginal pH. In addition to that, immune reactions in the vagina also influence vaginal ecosystem and play an important role in overall defence.

In the past decade, there were multiple studies of vaginal flora, including studies within the project of human microbiome and bacterial genome. The vaginal flora is ever changing; the quantities and types of vaginal microbiota may rapidly change within months to sometimes days.

Another important element that plays a role in vaginal micro-ecology is the functional condition of the multi-layered squamous necrotized epithelium of the vagina. The fluctuating levels of sex hormones, such as oestrogens, cause proliferation and maturation of vaginal epithelium. In vaginal atrophy, there is decreased efficacy of the barrier function of the epithelial layer of the vagina and, as a result, pathogenic organisms can easily penetrate into it.

The risk factors of impaired vaginal ecosystem may be endogenous (changes of hormonal levels, contraceptives, menstruation, pregnancy and lactation, diabetes, systemic disease) or exogenous in nature.

The exogenous factors may be lifestyle-dependant (smoking, stress, unprotected intercourse, etc.), infection-dependant or caused by medical interventions (antimicrobial therapy, radiation therapy, etc.).

Vaginal dryness

Vaginal dryness (atrophic vaginitis) is an irritation of vagina, caused by thinning and wrinkling of the tissues in vaginal walls, as well as by decreased secretion of natural lubrication. The dryness of vaginal mucosa produces discomfort, manifests as pain during sexual intercourse, bleeding of the vaginal wall (due to microtrauma caused by sex without sufficient natural moisture) and as itching and burning. Besides, another symptom of this unpleasant condition is frequent and strong urge to urinate.

The amount of lubrication produced by vaginal walls is vastly different in each woman. This is perfectly normal. As a rule, the woman does not notice any moisture in her vagina, since that fluid is colourless and has no odour whatsoever. When the moisture is no longer produced or produced in small quantities, the woman begins to experience dryness.

Since vaginal walls are thinned and are not capable of producing sufficient amounts of glycogen (the nutritious media for lactobacteria) and retaining lactobacteria on the mucous membrane (the lactobacteria are situated in a parietal fashion, in proximity to vaginal mucosa), the colonies of lactobacteria die off and their numbers steadily decrease. Reduced numbers of colonies of lactobacteria in vagina provide an excellent environment for colonization with pathogenic flora and the resulting nonspecific colpitis. Vaginal dryness adversely affects sexual life of the woman, since the pain she experiences during sexual intercourse forces her to evade sexual contacts or leads to a situation when the woman has completely no wish to have sex.

Thus, we have an understanding that vaginal dryness (atrophic vaginitis) = vaginal dysbiosis. Therefore, the chain of abnormal reactions looks like follows:

The hormonal imbalance produces a failure of the regenerating component of vaginal ecosystem. As a result, vaginal mucosa is no longer protected against the factors, which diminish its regenerative and restorative capacities. The mucous membrane of the vagina becomes thin and dystrophic changes begin to appear. In this connection, lactobacteria may no longer function normally, the colony count of lactobacteria is reduced and production of lactic acid decreases. This means that the second component of vaginal ecosystem is compromised and its function may no longer be normal; pH values change and shift to the alkaline side, which is an additional adverse influence on vaginal mucosa. The growth of opportunistic flora is triggered and, subsequently, colpitis develops.
